• A photo or screenshot of your most recent prescription, or the name and phone number of a US pharmacy where we can verify your prescription
• A photo or screenshot of a kidney function test ( metabolic labs) from the past 12 months if need more that 30 days supply of medication
• A blood pressure reading from the past 30 days
• A heart rate (pulse) reading from the past 30 days
• A photo ID like a driver’s license or passport
• A quick visit with the provider to know about your symptoms and your health history
